Mission and Purpose

LeadingChoice Network is a member-owned, financially and clinically integrated group of 56 high performing, mentored and monitored senior care organizations in 38 WI counties. Our mission, aligning services to improve the delivery of care in partnership with health plans and health system as preferred providers, is a ‘one-stop shop’ model for an Accountable Care Organization. Members created the Network as an entity to take advantage of value-based purchasing with a goal of taking upside/ downside risk in capitation. All LeadingChoice members are required to maintain specific quality standards and submit hospital transfer data monthly. LeadingChoice currently provides elite triple-aim outcomes while continually seeking to improve outcomes by implementing evidence-based clinical protocols and best practices.
