Mission and Purpose

LeadingChoice Network is a member-owned, financially, and clinically integrated cooperative of 51 high-performing, mentored, and monitored senior care organizations in 36 WI counties. Our mission, aligning services to improve the delivery of care in partnership with health plans and the health system, is a ‘one-stop-shop’ model for payer contracting. Launched in 2016, members created the Network as a vehicle for value-based purchasing that incentivizes quality of care. All LCN members are required to maintain specific quality standards and submit quality outcome data monthly. LCN continually seeks to improve outcomes by implementing evidence-based clinical protocols and best practices. Available only to LeadingAge Wisconsin members, LCN’s team has successfully positioned members with advantageous and exclusive contracts across lines of business. In addition to contracting, LCN provides support to member’s clinical and business office staff via assistance, advice, and implementation on best practices, compliance, credentialing, prior authorization, and claims processing.
